The threaded hole looks like a rear rack support, 2 unthreaded holes look like disc brake support. What's that slide for under the chain stay? Then you have a reinforced hole[?] on the seat stay.

Thats for use if it had a coaster brake hub and wheel...That is where the coaster brake arm is anchored to.

Most disc brake mounts are around 50mm apart. The one I just worked on was 51.5mm on center.

So, it is no easy feat to make one happen with the sprocket in the way. Damien has done a fine job of putting it together.
